Authorization and Agreement to Terms of Submission

This Authorization and Agreement is binding upon the individual identified in the “Your Name (Digital Signature) Authorization” field, above, as well as upon the “Signatory Company”, if different from the “Your Name” individual. By completing this form, you confirm that you are the individual identified in the “Your Name” field, above, or someone who is legally-authorized to sign and submit the information on this form as an employee or agent of the “Signatory Company” or “Your Name” individual.

I understand and agree that the PGA is collecting the personal information above and may provide that information to third parties, including but not limited to PGA member-volunteers, for review and forwarding to SAG-AFTRA. By submitted this form you authorize the PGA, and/or third parties including but not limited to PGA member-volunteers, to contact SAG-AFTRA on your behalf to assist the Signatory Company’s request for the return of the SAG-AFTRA Performance Bond for the aforementioned project/show.

I hereby forever generally and irrevocably release the PGA and the PGA’s directors, officers, members, principals, employees, staff, volunteers, contractors, advertisers, representatives, affiliates, agents, and attorneys, and each of the foregoing parties’ respective successors, heirs and assigns, from any and all claims and causes of action of any nature seeking any remedy whatsoever, at law or in equity, arising from or related to the submission of the information on this form, the submission of any other materials or documents, and/or any decision, action, or inaction regarding the Performance Bond above.
